Scope:

HSO is developing a National Standard of Canada (NSC) outlining best practices for the co-design and delivery of an integrated, people-centred spinal cord injury (SCI) rehabilitation program. The CAN/HSO S3402:2022— Integrated People-Centred Spinal Cord Injury Rehabilitaiton Program is a revision of HSO’s private standard HSO S3402:2013 – Spinal Cord Injury Rehabilitation Services, and will present the latest evidence-informed requirements for SCI rehabilitation program delivery at any point of care and in any care setting, focusing on principles of people-centred care, coordination, and integration. Developed as part of an on-going collaboration between the Praxis Spinal Cord Institute and HSO, the aim of the standard is to meet the comprehensive rehabilitation needs of persons with lived experience of SCI across the continuum, improving quality of life through better functionality and limiting the experience of disability throughout the person’s lifespan, as recommended by the World Health Organization (WHO) Rehabilitation in Health Systems 2030 Call for Action.[1] The standard will enable health systems and their system provider organizations to co-design and implement an integrated SCI rehabilitation program that connects the holistic needs of people with lived experience of SCI, recognizing that these needs differ throughout the care continuum. It will provide system provider organizations and their specialized SCI rehabilitation care teams with evidence-informed best practices that enable open, flexible patient pathways for timely delivery of SCI rehabilitation services – when needed and as defined by people and communities through shared decision-making. To that end, principles of care coordination and care collaboration within and across care teams will be outlined, in addition to those of co-design and engagement principles. Project need:

In 2019, HSO and the Praxis Spinal Cord Institute launched Setting the Foundation for a Spinal Cord Injury Patient Journey Assessment Program – a pan-Canadian needs assessment that aimed to identify current gaps in SCI care delivery models, as informed by the SCI community across Canada, and to better understand the needs of people with lived experience of SCI, particularly with respect to the design and delivery of SCI care and support services (from inpatient, to rehabilitation, and re-integration into the community). The needs assessment also identified gaps in current SCI standards of care and their assessment program, highlighting the importance of revising current SCI standards so that they reflect people-centred care principles when designing and implementing SCI care and service delivery models, placing the emphasis on co-design based on the lived experience. Additionally, there was a clear need for an integrated, people-centred SCI rehabilitation program that enables more coordinated, comprehensive, and continuous delivery of specialized SCI rehabilitation service post-transition from an inpatient rehabilitation setting and back into the community. CAN/HSO S3402:2022– Integrated People-Centred Spinal Cord Injury Rehabilitaiton Program is responding to the gaps identified through the needs assessment. It is intended to provide health and social service organizations and their associated specialized SCI rehabilitation care teams with evidence-informed guidance on how to work collaboratively with one another and with people and communities to support continuity of care irrespective of where the care is being offered. HSO is convening a Spinal Cord Injury Technical Committee to lead the development of CAN/HSO S3402:2022– Integrated People-Centred Spinal Cord Injury Rehabilitation Program.
