Incident Investigation and Prevention
Scope:
This proposed new Standard will cover a wide scope of topics related to the integration of incident investigation and incident prevention programs as part of an Occupational Health and Safety Management System.
The following elements are proposed to be included:
- Organization and Assignment of Responsibilities
- Training and Competency Requirements for Investigators and Data Analyzers
- Incident Reporting and Communications
- Incident Response and Control
- Initial Assessment, Investigation Preparation, and Resources
- Conducting Investigations
- Analyzing of Data (causal factors, control gaps, trends, etc.)
- Corrective Actions and Implementation of Controls (prevention)
- Modification to Management Systems (policies, procedures, training, etc.)
- Ongoing Integration into OHS Management System (continuous improvement, measurement tools, etc.)
