Note: This NOI (originally published on February 25, 2020) was reissued by the responsible SDO on July 6, 2021 and July 12, 2022, and June 2023. An additional comment period of June 13 to July 5, 2023.will apply.

The Standard will address application of equipment and technologies, information management, and infrastructure required to support the continuum of care of patients transitioning to a home and community care setting. This includes equipment and technologies for emergency planning, communication and data entry, transfer, storage and maintenance. In addition, the mechanism for equipment selection, set-up, data management, application and maintenance will be outlined.

This Standard is intended to assist in adapting health care provision to technological changes, and in making investments that will improve efficacy of care while prioritizing the safety, security and privacy of both patients and staff and their personal health information.

Project need:

Project Need
Health data integration is required in order for there to be coordinated care of a patient throughout their journey in the health care system including: within the health care facility; in the transition from the health care facility to the home and community; within the home and community; and potentially from the home and community back to the health care facility. The adoption and implementation of technologies and data management tools is essential to ensure continuity and quality of care amongst health care providers operating in settings outside of the traditional health care facility. However, knowledge about the effective care continuum strategies for patients transferred from the health care facility to the home and community setting is not clear. The purpose of this standard is to provide a guidance to facilitate the adoption and implementation of technologies and processes to maintain the care continuum for patients in home and community care settings
